This policy describes the intended website and messaging privacy practices of WeighLess MD & Wellness. It applies to this website and information voluntarily submitted through it. It is not a medical Notice of Privacy Practices and does not replace notices or consents provided during clinical care. This template does not currently deliver or store information requests.
2. Information you choose to provide
Once connected for live use, the information-request form is intended to collect your name, email address, optional mobile phone number, and separate messaging consent choices. Records of consent may include the disclosure shown and the time of consent. Do not submit medical records, diagnoses, payment details, or other sensitive health information through this general inquiry form.
3. How information is used
Information provided through a live inquiry form would be used to respond to requests, explain services, provide requested follow-up, and administer the messaging programs you separately select. It would not authorize unrelated marketing. Hosting providers may process technical connection information needed to operate and secure the website; actual providers and retention practices must be confirmed before publication.
4. Text messaging and your choices
If messaging is enabled, you may separately opt in to non-marketing service information updates, general inquiry follow-ups, and treatment-related clarification messages, or promotional messages about medical weight loss, wellness education, nutrition coaching, Invisa-RED, and hormone replacement therapy. Consent is optional and is not a condition of purchase. Neither checkbox is preselected. A phone number alone does not authorize SMS. After an opt-out, a final confirmation may be sent and no further messages will be sent under that consent unless you opt in again.
5. Sharing and mobile information
Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with third parties or affiliates for their marketing purposes. Service providers may process only the information necessary to provide requested services, including message delivery, under confidentiality obligations. Information may also be disclosed when legally required or necessary to protect rights and safety; this does not authorize marketing use of SMS consent data. Personal information will not be sold.
6. Cookies and external services
This template does not add advertising trackers or optional analytics cookies. Hosting or third-party services may use technologies needed for their own operation. Booking and directions links take you to independently operated services, including Practice Better and Google Maps. Their privacy policies govern information you provide to them. Any future analytics or advertising tools require this policy and applicable consent controls to be updated.
7. Retention and security
For live operation, information should be retained only as needed to handle inquiries, document consent and opt-outs, and meet legal obligations. Appropriate safeguards should be used, but no internet transmission is completely secure. The operator must verify its retention schedule and safeguards before launch. This preview does not send form entries to a server or save them in browser storage.
8. Privacy requests and children

This general website and its intended SMS programs are not directed to children under 18. Do not knowingly submit a child’s personal information through the inquiry form. Any clinical care for minors is governed separately.
